Snapshots of scenes, signs, everyday occurrences which easily confuse (me at least). Signposting, user interfaces shouldn't make us work to understand what's required of us, they should hard wire directly into the part of the brain that enables us to function.
Some examples I've come across that somehow miss the mark.

Toilets at Tegel Airport, Berlin - where's the mens?
This toilet in Tegel airport gets me everytime. Note the sign for the ladies next to the door for the mens. It makes sense, however it simply feels wrong. Click on Image to enlarge.

How come the French instruction doesn't require an exclamation mark!

Skolepatrulje - 'Green' for go or stop?
Children here in Denmark get to control the traffic around schools, providing safe passage to children. Their batons consist of yellow-green fluorescent circles - wouldn't red for stop be more appropriate? Imagine the scenario where a driver approaches road works, held up by a builder holding a red sign, indicating 'stop' and a green sign signaling 'go'. Wouldn't this conflict with the instructions at the school crossing further down the street?
